How to Register

  1. Click the menu link above or use this link to get a copy of the Registration Form.  Adobe Reader is required to access the registration form.  Follow this link to get a copy of Adobe Reader:  Get Adobe Reader.

  2. Print and complete the Registration Form. Give your team a name (a unique name is better than "The Leafs" or "The Storm", but please keep it clean as this name will be posted on the scheduling board).

  3. Please register as a team of FOUR players, all having birth dates within the same divisional guidelines (generally). Those teams with members of different ages must play in the older division.

  1. Ensure a parent or guardian signs for each player.

  2. To register, enclose in an envelope:

    • Your completed Registration Form (signed by all parents or legal guardians with email clearly printed)
    • A cheque for $80.00 payable to:
      • "RAY SCAPINELLO ROAD HOCKEY TOURNAMENT"
    • And mail or deliver to:
      • RAY SCAPINELLO ROAD HOCKEY

  3. Each team is required to bring a GAME KEEPER (GK) as a member of their team for each game they play. This GK must be an adult. He or she will be paired up with the other teams GK to KEEP the game running cleanly, KEEP time and KEEP score. They will be responsible for being familiar with the rules that are outlined in this brochure. A Scapinello Co-coordinator will be provided at each site to outline the rules before each game and to record the final score.

  4. Each team MUST have one GAME KEEPER for each game they play. Please indicate by checking the box on the registration form that your team understands this requirement.

  5. Only fully completed registrations will be accepted. Registrations will be accepted until April 17, 2010 or until full (we have not set a registration limit - in 2009 we had 135 teams). An early-bird prize will be drawn from entries received on or before April 3, 2010 (a team prize to be determined). To be assured a spot and to help us plan the event, please register early.

  6. A RAY SCAPINELLO ROAD HOCKEY TOURNAMENT co-coordinator will contact the Team Captain in late April regarding all tournament schedule details. All registered teams will receive an email or a written confirmation of their acceptance.
